Permissions Required: You need the Admin permission to use these functions.

 

Go to Admin Evaluations Patient History to customize the patient history items available for your clinic to use in evaluations, progress notes, and re-assessments.

Add a History Item

 

  1. In the lower right, click the + History Item button.
  2. Fill in the information for this history item.
  3. Click the Save History Item button.

Edit a History Item

 

  1. Click the history item you want to edit.
  2. Once you've made changes, click the Save History Item button.

Delete a History Item

 

You can only delete a history item if it has not been used in any documents.

 

  1. To the right of the history item, click the delete icon.
  2. Click the Delete History Item button.

About the Fields

 

Field Description
Name The name of the history item, which will appear on documents when selected by the therapist.
Default Default history items will automatically be selected for new documents.
