Overview

The Screening for Depression and Follow-Up Plan (CDF-AD) measure evaluates the percentage of adult clients (ages 18 and over) who are screened for depression using a standardized tool during an outpatient visit, and if screening is positive, have a documented follow-up plan.

Measurement Parameters

Population: Adults aged 18 or older as of the end of the measurement year.
Measurement Year (MY): January 1 - December 31 (12-month calendar year).
Encounter Window: Screening must occur on the date of the encounter or within 14 days prior.
