  1. Go to Day to Day 🡪 DCF FASAM Submission.
  2. Select the File Format being submitted using Patient data.

    • Client Data- Client demographic data must be submitted for all individuals receiving substance abuse and/or mental health services whose cost of care is funded, in whole or in part, by DCF funds, e.g. SAMH, TANF, Local Match, Title 21. Client data must be submitted prior to any other data submissions associated with that client. This creates a parent-child relationship between the Provider Client record and all other records.
      • Select a specific Program or leave blank to search All Programs.
      • Select a specific Patient and select Add to add their data to the table.

    • Treatment Episode- Treatment Episode data must be submitted for all individuals who meet the criteria for priority population and are eligible to receive substance abuse and/or mental health services whose cost of care is funded, in whole or in part, by DCF funds, e.g. SAMH, TANF, Local Match, Title 21. Treatment Episode data must be submitted prior to submitting service events. Also, the client demographic information and the provider information must already exist in FASAMS.
      • Select a specific Program or leave blank to search All Programs.
      • Select a specific Patient.
      • Select a specific Chart and select Add to add that charts data to the table.

    • Service Event- There are two types of Service Events to be submitted as specified below. Any entity (e.g., Managing Entity or Provider) that has a state contract to provide client-specific or non-client-specific services in community mental health or substance abuse programs is required to submit Service Event data.
      • Select a specific Program or leave blank to search All Programs.
      • Select a specific Patient.
      • Select a specific Chart.
  3. Client Specific Service Event (Visit) is a service encounter provided to individuals who meet the criteria for priority population and is eligible to receive substance abuse and/or mental health covered services and whose cost of care is funded, in whole or in part, by DCF funds (e.g. SAMH, TANF, Local Match, and Title 21).
    • Select a specific Service Event from this patient’s chart and select Add to add that event to the table.

  4. Non-Client Specific Service Event is a service encounter pertaining to substance abuse and/or mental health covered services whose cost of care is funded, in whole or in part, by DCF funds (e.g. SAMH, TANF, Local Match, and Title 21) and is provided to a group of individuals for whom individual client records are not maintained, or as part of an activity where client contact is maintained through an activity log as opposed to an individual client record.
    • To generate Non-Client Specific Service Events use the DCF EVNT Screen found under Day to Day 🡪 DCF EVNT.
    • Select the DCF EVNT and select Add to add the Non-Client Specific Service Event to the table.

      • Click Refresh once you have selected your File Format and applied any desired filters.
        • This will populate the table below.

  5. Select the Patient(s) to include in the file submission.
    • Check Select All to select all Patients in the table.
    • Check Unselect all to unselect all Patients in the table.
  6. Check Test Submission then click the Process button to test for any errors.
  7. Fix any errors and run a Test Submission again to ensure the errors have been resolved.

NOTE: this example is using the Client Data File Format.

  1. Once you receive no errors from the Test Submission, unclick the Test Submission checkbox and click Process to create the DCF FASAM File.
    • This file has been sent to the File Transfer Portal.
