Overview
Form Manager is used for the:
- Creation of charts and chart structures
- Setup of standard Release Forms
- Review of all forms in the Form Library
- Creation of simple (text / signature) Custom Forms
- Setup of Medication Administration Records (MARs)
Add and Remove a Section
- Go to Administration → Form Manager.
- Select the Program (Left side).
- Click Set Active to create the first active chart structure.
- If there is already an active chart structure, skip step 3.
- Click (Design) to assign the desired forms to the chart structure.
- Click Add Section to create a section for forms to be assigned to.
- It is common to have multiple sections to group forms by type, such Progress Notes, Consents, Assessments.
- Select a section that has already been created from the list.
- Click New to create a new section.
- Enter a Section name.
- Select a color for the Section.
- Click Save to add the Section to the Section List.
Exit the Section List Screen to begin assigning your forms to the section.
NOTE: To Remove a Section, select it in the Current Chart Structure and click Remove Item.
Click OK to confirm you want to remove the section.
NOTE: Click Save if you are not making any further changes.
Add Forms to a Section
- Search for forms in the Forms List and drag and drop them into the correct program in Current Chart Structure.
A. Note: Select a form in the Current Chart Structure and click Remove Item to remove it from the section.
Click OK to confirm the removal of the form.
- .Click Save to changes in the Chart Structure Designer.
- Note: this DOES NOT apply your changes to the active chart structure.
- Click Set Active to save your changes and apply your changes to the active chart structure.
NOTE: In Form Manager there is an additional Chart Structure every time you click Set Active.
